This contains all the forms that we would normally give you when you come into our office, but now you can get a head start in the comfort of your home.
We accept most major insurances, as well as Medicare and Medicaid. Flexible payment options are available upon request.

Direct Access! No Referral Needed.
Under South Carolina Direct Access laws, you can be seen at ELITE for up to 30 days without a physician’s script. That said, we still like to include your primary care physician and other medical professionals of your choosing in case we have to operate outside of that 30 day window.
