Physical, occupational and speech therapy all mutually focus on targeted tasks and problem solving to promote success in the home environment. However, each one can then be further broken down into assisting with the following objectives:
Physical Therapy:
Functional mobility, rehabilitation, assistive device utilization and balance.
Occupational Therapy:
Activities of daily living (bathing, dressing, driving), returning to work and fall prevention.
Speech Therapy:
Speech & language, cognition (memory, problem solving, safety), feeding/swallowing, fluency and voice disorders.
Upon receiving a scheduling request, whether from a physician referral or from the patient directly, administrative personnel will coordinate with a home visits therapist to contact the patient to schedule. They will arrange a two hour window in which the therapist will be expected by the patient to arrive; additionally the therapist will call the patient day-of when they are in route.
During the first visit, general paperwork and insurance benefits/financial responsibility will be reviewed. The first part of the evaluation will be spent discussing medical history, symptoms and goals. Various tests will be performed to determine functional capabilities and limitations. Results are then used to create a unique, individualized treatment program.

Elite Integrated Therapy Centers offers therapy services in the home and assisted-living facility settings through our Elite Home Visits program. In contrast to traditional Home Health, EITC Home Visits bills Medicare Part B providing valued continuity of care for patients discharging from Medicare Part A treatment plans. Our unique practice model in EITC Home Visits serves as an effective home health discharge plan that assists in reducing re-hospitalization/readmission rates for vulnerable patient populations.
All insurances are accepted and there is no physician referral required to begin treatment with us.
